Assessments

Assessments can improve clinical evaluation, inform diagnosis and guide therapy.  Here are several resources.

ADVERSE CHILDHOOD EXPERIENCES (ACE)

The ACE questionnaire measured 10 types of childhood trauma.  Five are personal and five are related to other family members.  The higher the ACE score, the higher your risk of health consequences.

AUTISM

DEVELOPMENTAL INVENTORY

The Developmental Inventory looks at adult behaviors and what developmental stage they originated.

The DSM-5 provides several free assessments for you to print and use in therapy.

  • Level 1 Cross-Cutting Symptom Measures - measure several different issues to determine areas for further testing.

  • Level 2 Cross-Cutting Symptom Measures - are specific for different issues

    • Depression

  • Personality Inventories - similar to the MMPI, these test for several abnormalities and/or personality disorders.

  • Early Development and Home Background - forms for parents and clinicians.

PROJECTIVE DRAWINGS

As counselors and mental health professionals we do not give projective assessments to diagnose.  We use projective techniques to inform therapy.  Projective drawings use specific prompts for the client to draw.  The emphasis is not on the quality of the drawing, but rather the placement, emphasis, inclusion, omission, or minimization of different attributes of the drawing.​

STRESS, DEPRESSION, ANXIETY

There is one spreadsheet that contains all three assessments that will calculate the score for each.  The others can be printed and hand-scored.

ADHD

There are several official ADHD assessments for children but fewer for adults.  Many adults have coped for years without an official diagnosis.  Here are some assessments to help in determining ADHD symptoms and intensity level.

There are several assessments on this page that help in vocational choice and development.

DISSOCIATIVE EXPERIENCES SCALE

The Dissociative Experiences Scale measures different types of dissociation - problematic and normal. 

MENTAL STATUS EXAM

A mental status exam can be done by observations of clients.  A Mini Mental Status Exam is more extensive if there is concern/uncertainty about cognitive functioning.

PERSONALITY ASSESSMENT

Personality tests can help with self-esteem, career choice, relationship issues, and more.​

Trauma Health Questionnaire (THQ)

The THQ asks questions about serious or traumatic life events. These types of events actually occur with some regularity, although we would like to believe they are rare. The questionnaire is divided into questions covering crime experiences, general disaster and trauma questions, and questions about physical and sexual experiences.
