Supervision

I have provided supervision for students and LPC-Interns/Associates for the past 12 years. I enjoy sharing my knowledge and experience working with diverse clients and issues. I am an approved Supervisor with the Texas BHEC State Board of Examiners of Professional Counselors. In Texas, you must complete 3000 hours and a minimum of 18 months under supervision an average of 4 hours a month. Here are some tools that will aid in the supervision process.

Counseling Hours Log

This log is based on the information the LPC Board wants to have on file while completing the 3000 hours of supervision as an associate. You will log your hours per week and briefly what was discussed in supervision. The form will calculate the hours automatically. There is a sheet for each site and a summary page with total hours.

Sample Session Note

I like to use a SOAP note because it is simple and provides enough information without being too specific.

S - Subjective - what the client reports
O - Objective - what the counselor does and observes in session.
A - Assessment - how the client responded to the treatment
P - Plan - what is your plan moving forward with therapy
